VisuMax LASIK vs. MEL90 Laser: Which Is Right for Your Eyes?

VisuMax is a femtosecond laser system developed by ZEISS. It is primarily used in the first step of certain refractive surgeries, such as LASIK and SMILE. Instead of using a mechanical blade (called a microkeratome), VisuMax uses femtosecond laser pulses — bursts of laser light lasting only a quadrillionth of a second — to make precise incisions in the cornea.
In the context of LASIK, VisuMax is used to create a flap in the outer layer of the cornea. This flap is then lifted so the inner corneal tissue can be reshaped by another laser. In the SMILE procedure, VisuMax goes even further: it creates a small disc-shaped piece of tissue inside the cornea (called a lenticule) and removes it through a tiny incision, with no flap involved at all.
VisuMax is known for several standout features:

The MEL90 is an excimer laser, also developed by ZEISS, and it is used in the second stage of LASIK. After the flap is created (often with the VisuMax), the MEL90 reshapes the cornea by removing microscopic amounts of tissue using ultraviolet light. This reshaping corrects the refractive error that causes nearsightedness, farsightedness, or astigmatism.
Excimer lasers have been used in LASIK for decades, but MEL90 represents a newer generation. It features advanced algorithms for smoother ablation patterns, faster treatment times, and greater preservation of corneal tissue.

SMILE (Small Incision Lenticule Extraction) is a flapless, minimally invasive alternative to LASIK. In SMILE, the entire correction is performed with the VisuMax femtosecond laser. A small lenticule of tissue is sculpted inside the cornea and removed through a tiny 2–4 mm incision.
Because there is no flap, SMILE offers unique benefits:
Reduced risk of dry eye after surgery
Better preservation of corneal strength
Smaller incision leads to faster healing
Fewer post-operative complications in certain patient groups
SMILE is not suitable for every patient — it has limitations in correcting very high astigmatism, for example — but for many, it offers an attractive alternative to LASIK with equal or better outcomes.
